Who
Can Attend?
Montgomery Township Residents who are over age 65 or who have chronic
illnesses (such as asthma and other respiratory illnesses, heart disease,
liver or kidney disease, and any other chronic illness that has an
effect upon the immune system).

What
Should I Bring to the Clinic?
If you are over 62 or otherwise eligible for Medicare, please
bring your Medicare Card with you when you come to the clinic.
If you do not have a medicare card, please bring your insurance
card with you to the clinic. We now have some cooperative agreements
with health maintenance organizations so that we can bill them directly.
You
should also bring identification to prove that you are a Montgomery
Township resident.
If
you are a Montgomery Township resident who is under age 65 and you
have a chronic illness, you should bring a note from your physician
indicating that your illness compromises your immune system such that
you require the influenza/pneumococcal immunization. Because
supplies are limited, a note saying it is all right for you to receive
the immunization will not be sufficient, and we do not keep such notes
on file from year to year.
